Your Personal Data

 

Here at LifeSolutions we take your privacy seriously. Any e-mailed enquiries send to us will be answered promptly, either by the owners (Alan Cooper and Carole Wan) or our administrator, Yvette Lowery. Your personal details will not be kept electronically unless you require an e-mailed reply. All emails will be deleted promptly. All records are kept in paper format only. If you make an appointment to see one of us (or both of us together in the case of Relationship Counselling), the following details will be requested: You will complete an intake form that will include your personal details, basic health information and details of your GP. We will also ask for your email for the purposes of providing a copy of any hypnosis session recorded for you. All recordings and copies of such will be destroyed six months after we conclude our work together, unless you request otherwise. In certain circumstances, we will, with your consent, forward certain information to your GP and/or other relevant health care practitioners.

 

Due to the nature of our work, we are required to securely hold your records for a period of ten years following your first appointment. Any records are paper records only and will be kept in secure fireproof cabinets. (Please see below for anyone under the age of 16). You will be asked to sign a contract that explains confidentiality and this will also be filed as above. You may request a copy of this at the time of signing.

 

Any payments will be recorded and will be forwarded to our accountant, on an annual basis. They will be identified by initial and surname only.

 

In the case of children, we are required to keep any records until he/she is 26 years old. The issue of consent will be discussed and recorded at the initial visit. As a general rule, children over the age of 13 may provide their own consent and under that age will require the written consent of a parent or guardian. At LifeSolutions, we choose to request consent for any child under the age of 18.
